Job summary

Cosán Group is hiring a Care Coordinator to work remotely from a home office compliant with HIPAA. You will manage a panel of patients, provide health coaching, and collaborate with clinical teams to close care gaps.

Ideal candidates have CMA or CPhT credentials, 1+ year clinical experience, and proficiency with EMR systems. The role requires US work authorization and a 50 Mbps/5 Mbps internet connection.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
• Must be legally authorized to work in the United States.

Responsibilities

• Outreach & health coaching: Manage 400–450 patients and conduct monthly outreach.
• Perform 50–60 daily outbound calls with care plans and health coaching.
• Maintain high-quality, professional communication in every interaction.
• Coordinate with clinical teams to identify and address needs.
• Support monthly outreach targets and care coordination goals.
• Document interactions in real time within the platform and ensure HIPAA compliance.
