Cosán Group is seeking a compassionate Care Coordinator to manage a panel of 400–450 patients from a 100% work-from-home setup in the United States. You will conduct monthly outreach, provide health coaching, and collaborate with clinical teams to close gaps in care while maintaining HIPAA-compliant documentation.
The role requires a CMA/RMA/CCMA/NCMA/CMAC or CPhT certification, at least 1 year of clinical experience, and proficiency with EMR systems and multiple technology platforms.
This is a work from home position, it is not mobile. Candidates must have a private, stationary, and HIPAA-compliant workspace within their home that is dedicated solely to work. Candidates must reside in the United States to be considered.
