Job summary

Ez Health, fully remote in the United States, is seeking a Patient Advocate to support older adults by coordinating appointments, medications, transportation, and benefits. You will translate medical terms into plain language and advocate for practical solutions.

You will work with a small caseload, building trusted relationships while handling outbound calls, documentation, and follow-ups to ensure each client’s needs are met. This role offers flexibility and growth opportunities.

Qualifications

• Clear communication, good judgment and empathy are essential.
• History of following through and getting things done.
• Comfort with email and online collaboration tools.

Responsibilities

• Calling clients on a regular schedule to check in and see what's new.
• Asking good questions and listening carefully to understand what each person really needs.
• Getting clients ready for upcoming appointments and walking them through follow-up steps.
• Translating medical terms, benefits, and next steps into everyday language.
• Coordinating with doctors' offices, pharmacies, insurance companies, and community programs.
• Setting up transportation, medication refills, meal assistance, and other practical help.
• Researching options and finding workarounds when a client runs into a roadblock.
• Documenting each call, the client's needs, and agreed-upon next steps.
