• Paralegal experience, JD, or similar degree from accredited program

Responsabilités

• Provide coaching, care coordination, referral and appointing services to clients enrolled in the VA Caregiver Legal and Financial program.
• Provide services via phone, text and video and a case management system for case notes, appointing, and other documentation.
• Assess needs of participating clients to inform personal plans in legal information and benefits navigation.
• Track, monitor and document coaching sessions in the case management system.
• Take dictation for legal forms and provide remote coaching to support information gathering and resource access.
• Develop personal goal plans with clients and identify action steps to achieve them.
• Link clients to resources and make referrals to facilitate goal attainment.
• Complete ongoing training as required by customer and Revelations.
