Job summary

Revelations Counseling & Consulting LLC is seeking a remote caregiver support specialist to provide coaching, care coordination, referrals and appointing services to clients enrolled in the VA Caregiver Legal and Financial program.

You will document sessions in the case management system, take dictation for legal forms, and connect clients with resources while maintaining high standards of confidentiality and client-centered care.

Qualifications

• Paralegal experience or JD or similar degree from an accredited program.

Responsibilities

• Assess needs of participating clients to inform personal plans including legal information and benefits navigation.
• Track, monitor, and document all coaching sessions within the case management system.
• Take dictation for legal forms.
• Provide web-assisted audio or video coaching using motivational interviewing and solution-focused techniques.
• Help clients develop personal goal plans and action steps.
• Provide connections to resources and referrals to facilitate goal attainment.
• Complete ongoing training as required by customer and Revelations.
