Job summary

Revelations Counseling & Consulting LLC is seeking a remote paraprofessional to provide coaching, care coordination, and legal/financial program support for VA Caregiver clients. You will conduct sessions via phone, text, and video, maintain case notes, and prepare documentation in the case management system.

Qualifications

• Paralegal experience, JD, or similar degree from an accredited program.
• Ability to conduct coaching via phone/text/video using motivational interviewing and solution-focused techniques.
• Experience with case management systems and documentation.

Responsibilities

• Assess clients' needs to inform personal plans including legal information and benefits navigation.
• Track, monitor, and document coaching sessions and related services in the case management system.
• Take dictation for legal forms.
• Provide web-assisted audio or video coaching to help clients obtain information, resources, and assistance.
• Help clients develop personal goal plans and action steps to achieve them.
• Provide connections to resources and refer clients for needed services.
• Complete ongoing training as required by the program and Revelations.
