Job summary

Ennoble Care is seeking an experienced Advanced Practice Provider (NP/PA) to deliver remote palliative care across NJ, PA, GA, VA, OK, KS on a full-time basis. You will assess patients, order tests, prescribe treatments, and collaborate with a multidisciplinary team to craft individualized care plans for patients with chronic illnesses.

Qualifications

• Board Certified as NP or PA with state licensure (NJ/PA/GA/VA/OK/KS)
• Strong clinical skills to assess, diagnose and treat
• Positive demeanor and strong people skills
• Ability to work independently and manage patient panels
• 2–3 years in palliative, chronic care management, or primary care with geriatric focus

Responsibilities

• Provide high quality palliative care to patients in NJ, PA, GA, VA, OK, KS
• Perform physical examinations and develop care plans
• Communicate effectively with office staff and operations platforms
• Deliver patient-centered care to a defined panel
• Collaborate with care team for holistic plans of care
• Build trust with patients, families, and caregivers during visits
• Lead goals-of-care conversations and transition to hospice when appropriate
• Order diagnostic testing as needed
• Prescribe medications, supplies, and interventions as medically necessary
