Job summary

Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to deliver remote palliative care to patients in NJ, PA, GA, VA, OK, KS on a full-time basis. You will apply medical techniques under a physician and care for a diverse set of patients with chronic diseases.

You will perform physical examinations, develop care plans, collaborate with a multidisciplinary team, and lead sensitive conversations about goals of care and hospice transition.

Qualifications

• Board certified and licensed as an NP or PA in NJ, PA, GA, VA, OK, or KS.
• 2–3 years of palliative, chronic care management, or primary care with geriatric patients.
• Hospice experience is preferred but not required.

Responsibilities

• Provide high quality remote palliative care to patients in listed states.
• Perform exams, update histories, and devise care plans.
• Coordinate with office staff and clinical operations across platforms.
• Collaborate with the care team to develop interdisciplinary plans.
• Lead conversations about goals of care and transition to hospice.
