Requirements include board certification and licensure in NJ/PA/GA/VA/OK/KS, with 2–3 years in palliative or chronic care.
Qualifications
- Board-certified NP or PA with active licensure in NJ/PA/GA/VA/OK/KS.
- 2–3 years of palliative, chronic care, or primary care with geriatric patients.
Responsibilities
- Provide high-quality remote palliative care to a diverse patient panel.
- Perform assessments, develop care plans, and coordinate with care teams.
- Lead conversations about goals of care and advance care planning.
- Order diagnostic tests and prescribe appropriate medications.
