Job summary

Ennoble Care is seeking an experienced Advanced Practice Provider (NP/PA) to deliver remote palliative care to patients across multiple states. You will work under physician supervision, perform examinations, create care plans, and collaborate with the care team to ensure holistic patient management.

Requirements include board certification and licensure in NJ/PA/GA/VA/OK/KS, with 2–3 years in palliative or chronic care.

Qualifications

• Board-certified NP or PA with active licensure in NJ/PA/GA/VA/OK/KS.
• 2–3 years of palliative, chronic care, or primary care with geriatric patients.

Responsibilities

• Provide high-quality remote palliative care to a diverse patient panel.
• Perform assessments, develop care plans, and coordinate with care teams.
• Lead conversations about goals of care and advance care planning.
• Order diagnostic tests and prescribe appropriate medications.
