Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to deliver remote palliative care to patients across NJ, PA, GA, VA, OK, KS on a full-time basis. You will apply medical techniques under physician supervision and care for diverse patients with chronic diseases.

You will assess health, develop care plans, and collaborate with the care team to ensure holistic, patient-centered outcomes. A strong clinical background, independence, and geriatric experience are essential.

Qualifications

• Board-certified NP or PA with state licensure (NJ/PA/GA/VA/OK/KS).
• 2–3 years of palliative, chronic care management or primary care experience with geriatric patients.
• Ability to work remotely with patients and collaborate with care teams.

Responsabilités

• Provide high-quality care to palliative patients in multiple states (remote).
• Assess health via physical exams, histories, and planning care.
• Communicate effectively with office staff and clinical operations.
• Deliver patient-centered care to a panel of patients.
• Collaborate with the care team to develop holistic plans of care.
• Build trust with patients, families, and caregivers during visits.
• Lead sensitive conversations about goals of care and hospice transitions.
• Order diagnostic tests as needed.
• Prescribe medications, supplies, and interventions.
