Job summary

Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to deliver remote palliative care to patients in NJ, PA, GA, VA, OK, KS on a full-time basis. You will work under a physician supervisor to care for patients with chronic diseases.

The role requires board certification, strong clinical skills, and the ability to work independently. Competitive compensation, flexible schedule, and strong support from a clinical team are offered.

Qualifications

• Board Certified NP or PA with valid state certification (NJ, PA, GA, VA, OK, KS).
• 2-3 years of palliative, chronic care management or primary care experience with geriatric patients.
• Strong clinical skills with ability to assess, diagnose and treat.

Responsibilities

• Provide high quality palliative care to patients remotely in listed states.
• Perform physical examinations and develop comprehensive care plans.
• Communicate effectively with office staff across platforms.
• Manage a panel of patients with patient-centered care.
• Collaborate with care team to develop holistic plans of care.
• Lead conversations about goals of care and advance care planning.
• Order diagnostic tests as needed and prescribe medications.
