Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to deliver remote palliative care to patients across NJ, PA, GA, VA, OK, and KS on a full-time basis. You will apply medical techniques under physician supervision, caring for patients with chronic diseases and complex needs.

Qualifications

• Board certified NP or PA with applicable state certification.
• Active license in at least one required state.
• Experience in palliative or chronic care management preferred but not mandatory.

Responsibilities

• Provide high-quality palliative care remotely to patients in NJ, PA, GA, VA, OK, KS.
• Assess health through exams, histories, and develop care plans.
• Communicate across internal platforms with office staff and operations.
• Deliver patient-centered, holistic care to a panel of patients.
• Collaborate with the care team to develop interdisciplinary plans of care.
• Build trust with patients, families, and caregivers during visits.
• Lead goals-of-care discussions and transitions to hospice when appropriate.
• Order diagnostic tests as medically necessary.
• Prescribe medications and interventions as needed.
