Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to provide remote palliative care to patients across multiple states.

Join a dedicated team delivering compassionate, high-quality care to chronically ill adults in a flexible, full-time role with strong support and growth opportunities. You will assess health, develop care plans, order diagnostics, coordinate with care teams, and have conversations about goals of care and transitions to hospice.

Qualifications

• Board Certified NP or PA with licensure in NJ/PA/GA/VA/OK/KS.
• Strong clinical skills to assess, diagnose and plan care.
• Positive demeanor and strong people skills.
• Ability to work independently and solve problems.
• 2–3 years in palliative, chronic care management or primary care with geriatric patients.

Responsibilities

• Provide high quality palliative care to patients in NJ, PA, GA, VA, OK, KS.
• Perform exams, obtain histories, and create care plans.
• Communicate with office staff and clinical operations across platforms.
• Deliver patient-centered care to a panel of patients.
• Collaborate with the care team to develop holistic plans.
• Foster trust with patients, families and caregivers during visits.
• Lead conversations about goals of care and hospice transitions.
• Order diagnostic tests as medically necessary.
• Prescribe medications and other necessary interventions.
