APGUARD HOSPICE LLC is seeking a dedicated RN Case Manager to join our remote hospice team. The role focuses on coordinating patient care across settings, optimizing outcomes, and guiding families through palliative care with expert nursing practice.

Ideal candidates have an active RN license (California preferred), strong assessment skills, and ability to collaborate with physicians and care teams. Flexible scheduling and advancement opportunities are offered.

Qualifications

• Valid RN license in California or eligible to practice in CA
• Experience in case management is advantageous
• Strong knowledge of physiology and medical terminology
• Experience with geriatrics, behavioral health, or infectious disease management is beneficial

Responsibilities

• Coordinate patient care plans with multidisciplinary teams
• Administer medications and monitor for side effects
• Document changes in patient conditions and report observations
• Maintain accurate patient care records and discuss findings with supervisors
