Job summary

Devoted Health is seeking a Palliative Care Social Work Case Manager to provide compassionate, patient-centered psychosocial support for patients with life-limiting illness. You will conduct telephonic/video visits, develop individualized care plans, and collaborate with the care team to address barriers and support families.

Qualifications

• Master’s degree in social work and active license.
• Minimum of 2 years experience in palliative or hospice care.
• Certified hospice and palliative social worker credential, or willingness to obtain within one year.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Establish trust and build strong relationships with patients and families.
• Collaborate with clinicians, nurses, and care coordinators for patient-centered care.
• Conduct psychosocial assessments and create personalized care plans.
• Lead goals of care conversations and advanced care planning; coordinate hospice transitions when appropriate.
