Devoted Health is seeking a seasoned Palliative Care Nurse to deliver remote home-based care for patients with complex needs. You will perform care coordination, participate in rounds, and work within an interdisciplinary team to ensure high-quality, compassionate end-of-life care.

Qualifications

Unrestricted RN licensure; compact preferred.
Ability to obtain licenses in additional states within 90 days of hire.
Minimum 5 years RN experience, with at least 2 years in hospice or palliative care.
CHPN credential, or willingness to obtain within six months.
Willingness to flex between time zones.
Self-starter in a work-from-home environment.
Strong interpersonal and communication skills.
Active Basic Life Support (BLS) certification.
Preferred ELNEC or EPEC training.
Bilingual in English and Creole, Spanish, or Vietnamese (preferred).

Responsibilities

Engage with patients telephonically and/or via video for home-based palliative care.
Perform essential care coordination and case management tasks.
Participate in daily team huddles and weekly palliative rounds.
Maintain patient panel; conduct routine check-ins based on risk and status.
Collaborate with other Devoted Medical teams to provide integrated care.
Assist with triaging and responding to incoming calls from palliative patients.
Support palliative education initiatives for Devoted Medical employees.
Work with teammates to grow the program and support patients.
