Job summary

Community Hospice and Palliative Care seeks a Social Worker to provide psychosocial support to patients and families and participate in the IDG for coordinated care.

The role requires an MSW and at least three years of direct service experience in healthcare, offering counseling, care planning, referrals, and coordination across home care, long-term care, and assisted living facilities in Columbia and Baker Counties and surrounding areas.

Qualifications

• Master’s Degree in Social Work from an accredited school of Social Work.
• Three years direct service experience.
• Experience in supportive counseling in group and individual settings.
• Experience in provision of case management services.

Responsibilities

• Visits patients in hospice inpatient units, homes, long-term care, and assisted living facilities.
• Manages a caseload including advanced directives, funeral planning, and complex case management.
• Assesses financial needs and facilitates referrals to resources.
• Provides patient/family education and psychosocial support.
• Collaborates with NASW standards and participates in IDG meetings.
• Coordinates with ancillary therapeutic services and supports end-of-life care.
