Job summary

OptumCare is seeking an RN to join the Palliative Care team, performing home visits and coordinating care across settings. You may interact with patients transitioning from hospital stays, provide education, and connect families with resources like Home Health Aides or Meals on Wheels.

Qualifications

• Current, unrestricted NV RN license or ability to obtain permanent license.
• 2+ years of RN experience.
• Bilingual English/Spanish communication skills.
• CPR certification or willingness to obtain within 30 days of hire.

Responsibilities

• Provide continuum of quality care through close communication with patients in person or by phone.
• Educate on conditions, review medications, connect to home health or meals services.
• Visit patients at home to discuss goals and resources.
• Collaborate with a team to address patient and family needs, including bereavement support.
• Support families and relieve pain and distressing symptoms.
• Other duties as assigned.
