Job summary

OptumCare is seeking a Palliative Care Manager to provide expert palliative care support to patients and families, strengthening primary palliative care capabilities across clinical teams. You will lead advanced communication, coordinate care, and collaborate across interdisciplinary teams to ensure compassionate, patient-centered, goal-concordant care throughout the disease trajectory.

Qualifications

• Current, active RN license in the State of Washington.
• Current BLS certification or ability to obtain within 30 days of hire date.
• 3+ years of experience in physician's office, clinical, hospice or hospital setting.

Responsibilities

• Conducts home dyad visits.
• Conducts telephonic nursing follow up.
• Coordinate care across settings (primary care, specialty care, inpatient, SNF, hospice, home health).
• Educate and coach staff on serious illness communication and palliative care principles.
• Promote interdisciplinary collaboration and patient-centered care practices.
