Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care program to provide remote, telehealth-based care to patients in nursing homes and senior living settings. The team delivers integrated care across areas of physical, mental, and social health, ensuring a seamless patient journey.

The role focuses on coordinating care, performing assessments, and educating patients and caregivers, with opportunities for professional growth within the Optum network.

Qualifications

• Active and unrestricted license in the state of residence and Oregon; ability to obtain in other required locations.
• For Nurse Practitioners: Master of Science in Nursing or Doctor of Nursing Practice; board certification in family, adult, gerontology or related NP track.
• For Physician Assistants: PA degree, active PA license, NCCPA certification.

Responsibilities

• Coordinate and manage care for members between caregivers, facilities, hospitals, and providers.
• Conduct routine, acute, follow-up and comprehensive assessments.
• Manage members during acute events or sub-acute rehab episodes.
• Ensure accurate documentation with ICD-10 coding.
• Collaborate with interdisciplinary care team; provide counseling and education to patients and caregivers.
• Maintain secure telehealth sessions and ensure privacy.
