Job summary

Atrius Health, part of the Optum family, is seeking a Registered Nurse for a remote role in the Palliative Care department. The position offers 40 hours per week, 8:30 AM to 5:00 PM, with a focus on high-quality patient care and coordinated care delivery.

You will build strong relationships with patients and families, perform medication management, triage calls, and document thoroughly in the EMR while supporting the clinical team and quality improvements.

Qualifications

• Graduate of a state-approved school of nursing.
• Current, unrestricted Massachusetts nursing license.
• AHA BLS certification.
• Up-to-date with current standards of nursing care in specialty.
• Experience with EMR and basic computer skills.

Responsibilities

• Build therapeutic relationships with patients and families, provide education and manage care transitions.
• Deliver patient care and follow RN protocols, including medication administration and chronic disease management.
• Perform telephone triage and escalate concerns as needed, supporting care coordination.
• Document timely and accurate information in the EMR during patient visits.
• Contribute to quality improvement and mentor nursing staff.
