Job summary

UnitedHealth Group in Washington state seeks a Palliative Care Manager to provide expert palliative care support to patients and families, lead advanced communication, coordinate care across settings, and strengthen primary palliative care capabilities within clinical teams.

Responsibilities include home dyad visits, telephonic nursing follow-up, advance care planning, and education of staff on serious illness care, while maintaining patient-centered, goal-concordant care throughout the disease

Qualifications

• Current, active RN licensure in the State of Washington.
• BLS certification or ability to obtain within 30 days of hire.
• 3+ years of experience in a physician's office, clinical, hospice or hospital setting.
• Valid driver's license and reliable transportation for daily travel.

Responsibilities

• Conduct home dyad visits.
• Conduct telephonic nursing follow-up.
• Develop, implement, and adjust palliative care plans aligned with goals.
• Coordinate care across settings (primary care, inpatient, SNF, hospice, home health).
• Educate staff on serious illness communication and palliative care principles.
