Atrius Health, part of the Optum family, is seeking a Registered Nurse for a remote Palliative Care position in Massachusetts. This role supports a value-based care system across multiple locations and emphasizes collaboration to deliver high-quality patient care.

You will build strong patient and family relationships, perform care planning and follow-ups, and contribute to clinical quality improvement while maintaining high standards of documentation and safety.

Qualifications

• Graduate of a state-approved school of nursing.
• Unrestricted nursing license in Massachusetts.
• American Heart Association BLS certification.
• EMR experience or aptitude to master EMR systems.
• Computer proficiency with word processing and spreadsheets.

Responsibilities

• Build therapeutic relationships with patients and families; educate and manage care transitions.
• Deliver patient care per RN protocols, including med administration and chronic disease management.
• Conduct telephone triage and support the clinical team with care coordination.
