Job summary

Chapters Health System, Inc. seeks a Nurse Practitioner for remote, virtual serious illness care management across our continuum, including hospice and palliative services. The role spans home-based, facility-based, and inpatient settings, with pediatric and adult patients.

The ReNW NP collaborates with the medical staff, performs remote assessments, and prescribes medications and equipment as needed. The position requires state licenses and national certification with BLS by start date.

Qualifications

• Requires 3+ years of clinical experience in hospice, palliative, or acute inpatient care.
• Must hold a Master’s or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• Must have national certification, licensure, prescriptive authority for home state and others; Basic Life Support; compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote, virtual serious illness care management to patients and families.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Supports discharge planning and documentation in EMR.
