Chapters Health System, Inc. seeks an experienced Nurse Practitioner for remote, nights/weekends coverage in serious illness care management.

The NP will support home, facility, and inpatient settings across hospice and palliative services, collaborating with a multidisciplinary team. You will provide virtual assessments, prescribe medications as appropriate, and ensure care plans align with regulatory guidelines.

Qualificações

• Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other required states.
• Must hold and maintain Basic Life Support certificate and compact RN license in home state by date of hire.

Responsabilidades

• Provides fully remote virtual serious illness care management to patients and families across Chapters Health System.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medication and equipment as appropriate.
• Adjusts and supports the plan of care per organization policies and nurse practice acts.
• Maintains EMR access and documents interactions in the EMR promptly.
• Participates in QAPI activities and promotes continuous care improvement.
