Job summary

Graham Healthcare Group is seeking a Hospice Intake Coordinator to support patients by coordinating referrals, documenting eligibility, and scheduling evaluation visits. This role can sit virtually and requires a dedicated home office setup with reliable internet.

You will partner with field staff and leadership to verify coverage, coordinate with insurance payers, and communicate with referral sources. A High School diploma or GED and 2+ years of healthcare administrative experience are

Qualifications

• High School diploma or GED required.
• Two+ years of administrative experience in healthcare.
• Reliable internet and ability to work from home.
• Home health or Hospice intake experience preferred.
• Homecare Homebase experience preferred.

Responsibilities

• Serves as the first line of customer service for internal and external customers.
• Receives and documents patient referrals to hospice services, ensuring coverage criteria.
• Reviews referrals for appropriate care setting and communicates with care center.
• Gathers information to assist the clinical team in scheduling evaluation visits.
• Coordinates with insurance verification and payer authorization as needed.
• Communicates and follows up with referral sources for required documentation.
