Job summary

Mount Sinai Health System is seeking a Virtual Support Assistant to back frontline clinical staff delivering palliative care at home. You will coordinate outreach, consent, scheduling, and orders, acting as a liaison to ensure care delivery proceeds smoothly.

The role supports an interdisciplinary team, helps manage home-based census operations, and tracks enrollments and care activities. Strong organization, communication, and healthcare data-entry skills are essential.

Qualifications

• High school diploma or equivalent required.
• 1+ years healthcare experience.
• Facility with online ordering systems, data entry.
• Ability to work both day and evening shifts, rotating holidays, and on call.

Responsibilities

• Patient outreach, program education, consent and scheduling for enrollment.
• Works as a member of our interdisciplinary care team assessing and addressing the complex care needs of patients and families.
• Supports admission and ongoing home based census operations and follows up on orders to coordinate care.
• Provides administrative support for tracking enrollments and ongoing care activities (eg Appointment scheduling, discharge orders, hospice transitions, etc)
• Communicates plan of care and assessments to front line teams
• Participates in huddles/IDT meetings daily/weekly with the rest of the care teams
• Strong organizational and communication skills
• Documentation as required
• Performs other related duties as assigned
