Job summary

Mount Sinai Health System seeks a Nurse Practitioner specialized in palliative care to provide in-home and telehealth assessments, symptom management, and goals-of-care discussions for patients with advanced illness.

You will work within an interdisciplinary team to develop and oversee care plans, educate families, and coordinate with primary care teams. Candidates require NYS NP licensure, DEA/BLS, and 3+ years in palliative care, with ACLS preferred.

Qualifications

• Master's degree program designed to prepare an NP to diagnose, treat, and prescribe.
• New York State license with NP certification and current registration.
• DEA certificate and BLS certification required; ACLS preferred.
• 3+ years of palliative care/geriatrics experience or related field.

Responsibilities

• Conduct in-home and telehealth assessments with complex needs; diagnose, order and interpret tests, and align care with patient goals.
• Prescribe new medications and perform medication reconciliation.
• Demonstrate sound clinical judgement and evidence-based practice in symptom management and end-of-life care.
• Facilitate family meetings, goals of care conversations, and advance care planning discussions.
• Lead care plans in coordination with the interdisciplinary team.
