Job summary

The Mount Sinai Health System is seeking a Care Coordinator to support logistical patient care activities for home-based patients. You will coordinate orders, vendor deliveries, and services, while collaborating with the clinical team to ensure safe, timely care.

You will maintain care coordination records, participate in daily huddles, monitor orders in Epic, and support quality and patient privacy per HIPAA. This is a full-time role with strong communication skills required.

Qualifications

• Minimum one year of healthcare experience; three years preferred.
• Experience with care coordination among patients, families and clinical staff.
• Experience drafting organizational protocols and workflows.
• Experience working with EMR; strong telephonic and coordination organization skills.
• Ability to measure and communicate program successes and lessons learned.

Responsibilities

• Assess complex care needs with the interdisciplinary care team.
• Support admission and home-based census operations; follow up on orders.
• Provide administrative support for tracking care activities (vendor liaison, equipment pickup, scheduling).
• Communicate plan of care and assessments to frontline teams.
• Participate in daily/weekly huddles with care teams; contribute to quality discussions.
