Job summary

Qualifications

• Requires Master's NP education and NY State NP licensure
• National Board Certification in Advanced Practice Nursing required for NPs who bill for professional services
• Collaborative Practice Agreement with a physician or documented FPPE when applicable

Responsibilities

• Conduct in-home and telehealth assessments for complex patients.
• Prescribe and reconcile medications.
• Lead care plan oversight with the interdisciplinary team.
