Job summary

Mount Sinai Health System seeks a Care Coordinator to support remote home-based care coordination, including orders, vendor liaison, scheduling, and safeguarding patient privacy.

You will work with the clinical team, document interventions, monitor orders in EMR, and participate in daily huddles. Minimum one year healthcare experience; associate or bachelor’s degree; Spanish fluency preferred.

Qualifications

• Minimum one year of healthcare experience; three years preferred.
• Experience with care coordination, patients, families, and clinical staff.
• Experience with EMR systems and coordinating care activities.

Responsibilities

• Assess and address complex needs as part of an interdisciplinary team.
• Support admission and ongoing home-based census operations and follow up on orders.
• Coordinate with vendors, equipment pickups, scheduling, and discharge confirmations.
• Communicate plan of care to frontline teams and participate in huddles.
• Maintain operational database and ensure HIPAA compliance.
