Job summary

The Mount Sinai Health System is seeking a Care Coordinator to support logistical patient care activities for Mount Sinai at Home. You will coordinate with clinicians, vendors, and patients to ensure timely services and accurate orders while maintaining HIPAA compliance and quality metrics.

Ideal candidates will have at least one year of healthcare experience with care coordination, EMR proficiency, and strong communication skills.

Qualifications

• Experience with care coordination for patients and families.
• Experience drafting organizational protocols and workflows.
• Experience working with EMR, strong telephonic and coordination organization skills.
• Proven ability to identify strategies for measuring and communicating program successes and lessons learned.

Responsibilities

• Works as a member of our interdisciplinary care team assessing and addressing the complex care needs of patients and families.
• Supports admission and ongoing home based census operations and follows up on orders to coordinate care.
• Provides administrative support for tracking ongoing care activities (vendor liaison, equipment pickup coordination, appointment scheduling, discharge order confirmation, etc).
• Communicates plan of care and assessments to front line teams.
• Participates in huddles/IDT meetings daily/weekly with the rest of the care teams.
• Manages and maintains operational database including relevant care coordination and care delivery related metrics.
• Serves as a liaison between physicians, clinical staff, and vendors.
• Sets priorities, problem solves and uses proper judgment in difficult situations.
• Monitors Epic orders.
• Promptly answers calls from clinical teams.
• Maintains accurate records of interventions and safety events.
• Submits utilization and assignment report every shift to supervisor and site leadership.
