Qualifications

• Graduate of an accredited Nursing program; RN with NY license.
• 2+ years of experience managing medically complex patients.
• Experience in interdisciplinary care teams.

Responsibilities

• Assess and address complex care needs in patients and families at home.
• Conduct home visits to support medical management and coordinate care.
• Provide counseling to reduce anxiety and symptoms of advanced illness.
