Healthcare Outcomes Performance Company is seeking a remote Care Coordinator for the Orthopedic MSK Digital Care Team. The role supports daily inbound calls, referrals, emails, and tasks while assisting Care Managers and ensuring patients progress toward outcomes.

The position emphasizes documentation, collaboration with patients and clinicians, and adherence to HIPAA. Prior MA/CNA experience and familiarity with value-based care or orthopedics are preferred.

Qualifications

• MA or CNA degree or equivalent preferred.
• Minimum three years of CNA/MA experience preferred.
• One-year experience in medical office or hospital environment preferred.
• Experience in value-based care or orthopedics preferred.

Responsibilities

• Oversee daily inbound calls, referrals, emails for the department.
• Assist Care Managers with assigned tasks.
• Monitor patient progress and assess outcomes to ensure desired results.
• Identify and resolve barriers to care.
• Document clearly, concisely, and accurately.
• Collaborate with clinicians, patients, families, and team members to resolve issues promptly.
• Adhere to HIPAA and patient confidentiality policies.
