Job summary

Daymark Health is seeking an Oncology Nurse Navigator to join our Care Team. This full-time, remote role supports oncology patients via phone and virtual platforms, guiding them from screening through survivorship or end-of-life care.

You will triage symptoms, provide clinical and care coordination, educate patients about Daymark services, and collaborate with clinicians to deliver empathetic, patient-centered care while adhering to our documentation standards.

Qualifications

• BSN required with CA RN license and at least 3 years of nursing experience, including 2 in oncology.
• Oncology certifications (OCN/AOCN/AOCNS/CCM) strongly preferred or pursued within 2 years.
• Comfort with telehealth tools and virtual platforms.
• Ability to build rapport with diverse patients and care teams.
• Strong organizational and multitasking abilities.

Responsibilities

• Provide direct patient care via virtual platforms and phone/video consultations.
• Triage and manage symptoms with escalation to Nurse Practitioner as needed.
• Coordinate care, including referrals, discharge planning, and social supports.
• Educate patients about diagnoses, Daymark services, and next steps.
• Build trusted relationships with patients and families.
• Collaborate with clinical staff to ensure protocol alignment.
