Daymark Health is seeking a National Oncology Nurse Navigator to join our remote care team. This full-time role supports oncology patients across the United States via phone and virtual platforms, coordinating care with physicians, social workers, and enrollment specialists.

The ideal candidate holds a BSN, an unrestricted RN license valid in all 50 states, at least 3 years of nursing experience (2 in oncology), and comfort with Slack, Zoom, Google Suite, and EHRs.

Qualifications

• BSN with an unrestricted RN license and multi-state licensure in all 50 states.
• At least 3 years of nursing experience, including 2 years in oncology.
• Oncology certifications (OCN, AOCN, AOCNS) or CCM are preferred or to be obtained within 2 years.
• Active multi-state RN license and additional licenses in non-compact states (e.g., CA, NY, IL).
• Comfort using Slack, Zoom, Google Suite, and EHR systems.
• Strong relational, communication, and patient-centered approach.
• Bias to action, initiative, adaptability and flexibility for on-call expectations.

Responsabilités

• Provide direct patient care via virtual platforms and deliver high-quality oncology care.
• Offer triage, case management, and care navigation from screening to survivorship or end-of-life care.
• Triage symptom responses and escalate to a Nurse Practitioner when needed.
• Coordinate care with Medical Director, NP, Social Worker, and Enrollment Specialist.
• Educate patients about diagnosis and Daymark Health’s care offerings.
• Build rapport with patients and care networks; address social determinants of health.
• Ensure compliance with policies, documentation standards, and effective use of systems.
