Job summary

Point32Health is seeking a Care Manager - Nursing (Field) to ensure pregnant members receive comprehensive care management across transitions. This role suits experienced nurses with strong clinical knowledge and critical thinking skills.

The position offers remote work with MA residency, a full-time schedule, and competitive pay. The role requires RN licensure, a Bachelor's degree, and 5 years of relevant clinical experience, with CM certification desirable.

Qualifications

• Bachelor's degree required; relevant experience considered.
• 5 years of clinical experience required.
• RN license in state of residence.
• National Case Management certification desirable.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters using motivational interviewing skills.
• Conduct departmental assessments to identify member needs and barriers to wellness.
• Develop and implement member-specific care plans with members, caregivers, and providers.
• Complete documentation per policy and regulatory standards.
• Provide targeted health education and proactive condition management strategies.
• Collaborate with the facility care team for safe transitions to next care level.
• Liaise with interdisciplinary care team to improve outcomes.
• Present high-risk members at interdisciplinary rounds.
• Pursue professional growth through self-directed learning.
