Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters using motivational interviewing skills.

• Complete documentation per policy and regulatory standards.
• Provide targeted health education and proactive condition management strategies.
• Collaborate with the facility care team for safe transitions to next care level.
• Liaise with interdisciplinary care team to improve outcomes.
• Present high-risk members at interdisciplinary rounds.
• Pursue professional growth through self-directed learning.

About This Role

What You'll Do

• Perform telephonic member outreach and/or face-to-face encounters using motivational interviewing skills.
• Conduct departmental assessments to evaluate member needs, health goals, and barriers to wellness.
• Develop and implement member-specific plans of care in collaboration with members, caregivers, and providers.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Collaborate with the facility care team to ensure safe transitions to the next level of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.

Requirements

• Education: Bachelor's degree required, relevant equivalent experience considered.
• Experience: 5 years' relevant clinical experience required.
• Experience in home care or case management preferred.
• Licenses & Certifications: Registered Nurse with current unrestricted license in state of residence.
• National certification in Case Management desirable.
