Responsibilities

• Evaluates the member's needs, barriers to care, and social determinants to plan care.
• Develops ongoing care/service plans and coordinates with providers to address unmet needs.
• Identifies barriers to care and applies appropriate care management interventions.
• Coordinates between member/family and care team to ensure person-centered care.
• Provides follow-up and monitors progress toward goals; revises plans as needed.
• Offers resource support for local services (housing, employment, etc.).
• Facilitates access to needed care/services through collaboration with providers.
• Performs outreach via telephonic, digital, or home visits as required.
• Documents all activities to ensure regulatory compliance and accurate records.
• Complies with policies and performs other duties as assigned.
