Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• Minimum 5 years' relevant clinical experience.
• Bachelor’s degree or relevant equivalent experience.
• Preferred: Bachelor’s degree in nursing.
• Proficiency in a second language desirable.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Assess members to identify needs, goals, and barriers to wellness.
• Develop and revise member-specific plan of care with providers.
• Complete documentation according to policy and standards.
• Provide health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care with the care team.
