Job summary

Galileo Health is seeking a Nurse Practitioner to join our Population Health team. You will deliver post-hospital follow-up via phone and video, focusing on medication reconciliation, symptom screening and arranging necessary follow-ups, labs or medications.

Ideal candidates have CT licensure, 5+ years in primary care/geriatrics, and comfort with a digital-first care model. Flexible scheduling and weekend coverage are available, with competitive compensation.

Qualifications

• Nurse Practitioner in good standing with CT licensure or eligible for CT licensure.
• Minimum 5+ years clinical experience in Primary Care or Geriatrics.
• Experience engaging Medicare/Medicaid populations is preferred.

Responsibilities

• Provide telephonic outreach and scheduled video/phone visits for recently hospitalized patients.
• Perform medication reconciliation, symptom screening, and arrange follow-up tests or prescriptions.
• Build patient trust and explain Galileo’s care model to connect patients with services.
• Coordinate care across hospital, ED, pharmacies and community providers.
• Collaborate with the nursing team and contribute to population health goals.
