Job summary

Folioworks is seeking a Nurse Practitioner specialized in geriatrics to deliver telehealth longevity and comprehensive care for adults 65+. The role focuses on functional assessment, cognitive screening, chronic disease management, medication review, and advance care planning within a Medicare-billed model.

You will work under the supervision of a board-certified geriatrician, enjoy 100% remote scheduling, and contribute to care for underserved older adults while maintaining HIPAA-compliant

Qualifications

• Active, unrestricted NP license in New York.
• Board certification as AGNP/AGPCNP or Family NP with geriatric focus.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.
• Reliable high-speed internet and private HIPAA workspace.

Responsibilities

• Conduct comprehensive longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk.
• Provide medical follow-up visits for chronic disease management and ongoing care.
• Perform cognitive screening and staging, escalating for further assessment as needed.
• Prescribe and manage GLP-1 therapy for appropriate patients.
• Conduct advance care planning conversations.
• Document care in the EHR to support Medicare billing.
