Job summary

Folioworks is seeking a Telehealth Nurse Practitioner to provide longevity and geriatric visits for adults 65+, including initial assessments, chronic disease follow-up, cognitive screening, and advance care planning in a Medicare-focused remote model.

The clinician will document visits in the EHR for Medicare billing, collaborate with a geriatrician-led team, and benefit from clinical training and flexible scheduling as part of a remote, HIPAA-compliant workflow.

Qualifications

• Active, unrestricted NP license in Michigan.
• Board certification as Adult-Gerontology NP or Family NP with geriatric focus or demonstrated interest in aging.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.
• Reliable high-speed internet and a private, HIPAA-compliant workspace.

Responsibilities

• Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk.
• Provide ongoing medical follow-up visits for chronic disease management and longitudinal care.
• Perform cognitive screening and staging, and elevate for further assessment when indicated.
• Prescribe and manage GLP-1 therapy for appropriate patients.
• Conduct advance care planning conversations.
• Document visits in the EHR to support accurate Medicare billing.
