Job summary

Curana Health is seeking a Nurse Practitioner for a work-from-home, after-hours telehealth role serving aging residents across multiple states. You will provide clinical direction by phone, coordinate care with facilities and clinics, and document encounters in the EMR per CMS standards.

The ideal candidate has an NP degree, national certification (ANP/FNP/GNP), and 3+ years of NP experience in adult/geriatric care, with DEA prescriptive authority and multi-state licensure.

Qualifications

• Master's degree as NP
• Active licensure in multiple states (TX/LA/TN or others)
• NP national certification (ANP/FNP/GNP)
• DEA licensure / Prescriptive Authority
• Background in acute and chronic disease management
• Clinical background in adult, family, or geriatrics
• 3+ years NP experience
• Ability to obtain collaborative practice agreement
• Ability to work scheduled shifts
• Proficient with electronic medical records (EMR)

Responsibilities

• Serve as first line of support for residents and facility staff after-hours, providing direction and medical care over the phone.
• Coordinate care between facilities, hospitals, and clinics using Curana’s telephonic platform.
• Deliver high-quality, cost-effective care addressing acute, chronic, and behavioral health needs with physicians and specialists.
• Perform comprehensive assessments and document encounters in the EMR per CMS requirements.
• Apply Curana’s clinical protocols to support safe, effective treatment.
• Participate in mandatory education and training to stay current with standards of care.
