Job summary

Curana Health is seeking skilled Nurse Practitioners for our National After-Hours Call Team. This work-from-home role provides telephonic care and clinical direction to aging residents across multiple states, delivering high‑quality care without unnecessary transfers.

You will manage high call volumes, perform at least 30% telehealth visits, evaluate acute changes, falls, and controlled substance visits, and document encounters accurately in the EMR according to CMS requirements.

Qualifications

• Master's Degree as a Nurse Practitioner and NP national certification required.
• Licensure in NC and SC; other state licenses preferred; DEA licensure helpful.
• 3+ years of NP experience in acute or chronic care; geriatrics experience preferred.
• Ability to work remotely across multiple states and manage after-hours duties.
• Proficiency with electronic medical records (EMR) and CMS guidelines.
• Ability to establish collaborative practice agreements as needed.

Responsibilities

• Serve as first line of after-hours support for residents and facility staff via phone.
• Provide clinical direction and telephonic care; coordinate with facilities, hospitals, clinics.
• Deliver high-quality, cost-effective care addressing acute/chronic/behavioral health needs.
• Document encounters accurately in the EMR and ensure CMS compliance.
• Apply clinical protocols and participate in ongoing training to stay current.
• Manage high call volumes and contribute to continuous after-hours coverage.
