Job summary

Devoted Health is seeking a seasoned Transitions of Care Advanced Practice Provider (NP/PA) to lead post-discharge care for high-risk patients across the U.S. in a fully remote model.

You will perform initial TOC visits, reconcile meds, order tests, and coordinate with an interdisciplinary team to prevent readmissions. Required: 5+ years of geriatric patient care, licensure as RN/APRN or PA, and experience with telehealth.

Qualifications

• 5+ years of direct patient care managing primarily geriatric populations with multiple chronic, complex, comorbid conditions.
• Experience performing visits over telehealth video platforms.
• Ability to work full-time, five-8 hour days/week (Mon-Fri, 8:30 AM - 5:00 PM; CST/MST/PST preferred).

Responsibilities

• Perform comprehensive initial TOC visits and post-discharge assessments.
• Complete post-discharge medication reconciliation and optimize therapy.
• Place orders, provide education, and outline contingency plans.
• Independently assess and manage new or worsening symptoms in medically complex patients.
• Participate in interdisciplinary TOC rounds and quality-improvement projects.
