Job summary

Curana Health is seeking a Nurse Practitioner for our National After-Hours Call Team to deliver telephonic care after hours across multiple states, focusing on aging residents with timely access and appropriate escalation when needed.

You will work remotely with high call volumes, performing at least 30% telehealth visits and collaborating with physicians to provide high-quality care aligned with CMS requirements.

Qualifications

• Master's degree required and NP license active in at least 2 states.
• National certification as ANP, FNP, or GNP required.
• DEA licensure / Prescriptive Authority required.
• 3+ years of NP experience in acute/chronic disease management.
• Ability to obtain collaborative practice agreement where required.
• Proficient in documenting in EMR and coordinating care remotely.

Responsibilities

• Serve as the first line of after-hours telephonic clinical support for residents and facility staff.
• Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics.
• Deliver high-quality, cost-effective care addressing acute, chronic, and behavioral health needs with physicians and specialists.
• Document encounters accurately in the EMR and ensure CMS compliance.
• Apply Curana’s clinical protocols and guidelines to support safe, effective treatment.
• Participate in mandatory education and training to stay current with standards of care.
