Job summary

ComplexCare Solutions seeks a Nurse Practitioner per diem to perform comprehensive in-home assessments across Denver County, CO. You will spend substantial time with each member to capture needs, review conditions, and craft care plans to keep patients in the home and out of the hospital.

Role emphasizes CMS guidelines, HIPAA compliance, and collaboration with physicians and health plans. Remote-ready, with available telehealth options when appropriate.

Qualifications

• Active un-encumbered nursing license required.
• ANCC or AANP board certification as NP or CNS in relevant specialties.
• Maintains CPR certification and HIPAA compliance.
• Vaccination/testing per Healthcare Personnel requirements.
• Experience with Medicare/Medicaid benefit structures beneficial.

Responsibilities

• Conduct comprehensive in-home assessments with detailed documentation.
• Collaborate with physician and health plan, escalating urgent needs as needed.
• Document CMS/DHHS compliant medical conditions and treatment plans.
• Provide evidence-based recommendations to improve chronic condition management.
• Maintain patient confidentiality and accurate electronic records.
