Job summary
ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem home and telehealth assessments across Mesa County, CO. The provider will work remotely and conduct comprehensive evaluations, develop treatment plans, and communicate results to PCPs and health plans.
The role requires ANCC/AANP NP certification, an active nursing license, CPR, and HIPAA compliance. Prior home health experience and bilingual abilities are preferred; a flexible schedule supports approximately 37 hours per
Qualifications
- Active un-encumbered NP license
- ANCC or AANP board NP certification in Family/Adult/Gerontology/Emergency
- CPR certification
- Knowledge of CMS Regulations and NCQA HEDIS Guidelines
- Experience in home health a plus
- Ability to work autonomously in a remote clinical environment
- Excellent customer service and communication
- Bi-lingual a plus
Responsibilities
- Conduct comprehensive member assessments in home or telehealth visits
- Document findings and enter into EHR with timely follow-up
- Develop evidence-based treatment plans for chronic conditions
- Identify gaps in care and coordinate with PCP/health plans
- Ensure HIPAA compliance and patient confidentiality
