Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to perform in‑home and telehealth assessments across Sarpy County, Nebraska, with flexibility for remote work within the U.S. You will spend up to an hour with each member, documenting findings and developing care plans to keep patients safely at home.

Qualifications

Active, unencumbered NP license to practice in the United States.
ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
Current CPR certification.
Experience with CMS/NCQA guidelines and ICD-10/CPT-4 coding.
Ability to document thoroughly and communicate effectively in a remote care setting.

Responsibilities

Perform comprehensive in-home and telehealth assessments for members.
Document assessment findings and formulate evidence-based treatment plans.
Coordinate with PCPs and health plans and ensure timely follow-up.
Maintain HIPAA-compliant documentation and data security.
Address gaps in care and provide member education on chronic conditions.
